丙酮酸是三大营养物质代谢的中心,既往研究表明丙酮酸盐及其代谢物有缓解代谢、降血脂、降胆固醇、缓解疲劳等作用。而近年来研究表明在烧伤及休克等补液治疗时加入丙酮酸盐有改善血容性休克、感染性休克、缺血再灌注损伤、多器官功能障碍的作用,并成为抗休克治疗研究的新热点。丙酮酸盐的抗休克作用可能与抑制重要脏器血管通透性,减轻缺血再灌注损伤,保护心、肾、脑和肠道功能及纠正乳酸性酸中毒,降低各重要脏器中谷胱甘肽过氧化物酶的活性,抑制高迁移率蛋白-1,增加缺氧诱导因子-1水平的升高有关,本文就近年关于丙酮酸盐抗创伤、烧伤休克及脓毒性休克的相关研究进展综述如下。

Pyruvic acid is the center of the metabolism of the three major nutrients, the previous studies have shown that pyruvate and its metabolites have the effect of relieved metabolism, lowering blood pressure, cholesterol, relieve fatigue and so on. In recent years, studies have shown that at the time of the treatment of burn and shock and other rehydration therapy, the addition of pyruvate has improved hematocrit shock, septic shock, ischemia and reperfusion injury, multiple organ dysfunction. It has become a new hot spot in anti-shock treatment research.In recent years, the anti-shock effect of pyruvate may be related to the inhibition of vital organs vascular permeability, reduce ischemia-reperfusion injury, protect heart, kidney, brain and intestinal function and correct lactic acidosis, reduce the activity of glutathione peroxidase in important organ, inhibition of high mobility group box-1, increased hypoxia inducible factor-1 levels increased. In this paper, recent studies on pyruvate anti-trauma, burn shock and septic shock are summarized below.
