Guiding significance of staging of osteonecrosis of the femoral head blood supply disorder in the treatment of osteonecrosis of the femoral head

Abstract:

Osteonecrosis of the Femeral Head (ONFH) is a common clinical disease, mainly divided into traumatic osteonecrosis and non-traumatic osteonecrosis. Traumatic ONFH is mainly due to the rupture of the artery feeding the femoral head under the action of external force, which directly destroys the blood supply of the femoral head. The main causes of non-traumatic femoral head necrosis include corticosteroid application, long-term alcohol overdose, decompression sickness, hemoglobin disease, etc. Pathogenesis is divided into the following processes: intravascular coagulation, venous stasis, increased intraosseous pressure, arterial injury, ischemic necrosis of the femoral head. In vitro, micro computed tomography(Micro-CT) scan of isolated femoral head specimens infused with nano-barium sulfate have been used to establish a blood transport model. In vivo digital subtraction angiography (DSA) have been used to observe blood flow in femoral heads with different osteonecrosis. Combining the images performance and the clinical symptoms, the author puts forward the staging theory of blood supply disorder of ONFH and applies it to the diagnosis and treatment of ONFH at various clinical stages.
