肺部感染是烧伤患者最常见的并发症,也是其主要死亡原因之一。合并严重吸入性损伤的烧伤患者往往需要进行气管插管,这为病原微生物进入下呼吸道建立了通道。严重的烧伤会导致全身免疫抑制。吸入性损伤在破坏气道防御机制的同时,也会导致气道局部免疫抑制,这都可导致烧伤合并吸入性损伤患者发生呼吸机相关肺炎(VAP)。本综述通过目前呼吸机相关肺炎的研究结果,对烧伤合并吸入性损伤患者发生VAP的易感因素、治疗及预防进行总结,旨在为烧伤患者VAP的救治提供参考。

Pulmonary infection is the most common complication and one of the main causes of death in burn patients. Burn patients with severe inhalation injury often need endotracheal intubation, which establishes a channel for pathogenic microorganisms to enter the lower respiratory tract and severe burns can lead to systemic immunosuppression.Inhalation injury not only destroys the airway defense mechanism, but also leads to airway local immunosuppression, which create conditions for ventilator-associated pneumonia (VAP) in burn patients. This article summarizes the current research results of VAP, and reviews the susceptibility factors, treatment and prevention of VAP in burn patients with inhalation injury, in order to provide reference for the treatment of ventilator-associated pneumonia in burn patients.
