Abstract:
Objective To explore the application and nursing effect of vacuum sealing drainage after the skin flap trarsplantation of varicose ulcer of lower extremity.
Methods A total of 38 cases of skin graft after debridement of varicose ulcer in lower extremity enrolled Burn Trauma Center, Changhai Hospital, Naval Military Medical University from July 2016 to July 2017 were selected. According to the random number table method, patients were divided into control group and observation group, with 19 cases in each group. Patients in control group were given conventional dressing method, and the patients in the observation group were treated with vacuum sealing drainage therapy on the basis of conventional dressing method. Stress management, pipeline management, and basic nursing were also emphasized. The therapeutic effects of the two groups were compared, including average dressing times, hospitalization time, positive rate of bacterial culture of wound and the survival rate of skin graft, t test and chi-square test were used for statistical analysis.
Results The average dressing time of patients in the observation group was (2.53±1.17) times, it was lower than the control group [(5.05±1.90) times], the difference was statistically significant (t=4.93, P<0.05). In the time of hospitalization, the observation group was (8.11±1.97) d, it was relatively shorten than the [(10.47±4.29) d] of control group, the difference was statistically significant (t=2.19, P=0.038). The positive rate of bacterial culture in the observation group was 10.5%, compared with the [42.1%] of control group, the difference was statistically significant (χ2=4.89, P=0.027). The survival rate of skin graft in the observation group was (92.11±5.61)%, which was significantly higher than that in the control group [(87.33±7.88)%], the difference was statistically significant (t=-2.14, P=0.04). No adverse events were found in the two groups.
Conclusions After the free skin graft, vacuum sealing drainage therapy combined with good nursing support can shorten the treatment length, reduce the times of dressing change and the positive rate of bacterial culture. It is a relatively safe and effective assistant technique for repairing chronic wound.
